「Minimally invasive surgery for biliary diseases」

Aim of Session
 Recent revision of Japanese insurance system for medical treatments has allowed us
to perform minimally invasive surgery( MIS) for various benign and malignant biliary diseases.
On the other hand, because of high-level difficulty of biliary operation even in open
approach and strict facility standards, MIS for biliary diseases can be performed only in
limited institutions. Therefore, there are still lots of controversies in this field such as
indication of choledocholithotomy, application to type IVA bile duct dilatation, indicated
stage of gallbladder cancer and adequate resection range, application to hilar bile duct
cancer, and reliable method of biliary reconstruction( interrupted vs. continuous suture), etc. In this session, presentations related to current situation and future perspective of MIS for various benign and malignant biliary diseases using technical video and outcome data will be welcome.

「Best techniques for EUS-guided biliary drainage(EUS-BD)」

Aim of Session
 EUS-BD has become widely used as a new drainage technique for the biliary tract,
including hepaticogastrostomy (EUS-HGS), choledocoduodenostomy (EUS-CDS),
hepaticoduodenostomy (EUS-HDS), and gallbladder drainage (EUS-GBD). In addition, the usefulness of applied EUS-BD techniques such as rendezvous technique and antegrade intervention, has also been reported. Basically, the EUS-BD procedure consists of 4 steps: puncturing the bile duct, introducing the guide wire, dilating the digestive and bile duct wall, and deploying the plastic or metal stent. At each step, there are factors that can lead to procedural difficulties or failures. To avoid fatal or serious adverse events, it needs to be managed in case of troublesome situations during the procedures.
In this session, we would like to discuss the most appropriate techniques for safe and reliable EUS-BD.
